RESUMO

Objective To investigate the effect of exposure to lead oxide nanoparticles (PbO NPs) on the polarization of microglia in mouse hippocampus. Methods i) Specific pathogen-free male C57 mice were randomly divided into control group, low-, medium- and high-dose groups, with 10 mice in each group. Mice in these three dose groups were intraperitoneally injected with PbO NPs suspension at doses of 5, 10 and 20 mg/kg per day, respectively, and mice in the control group were intraperitoneally injected with the same volume of 0.9% sodium chloride solution, five days per week for four weeks. ii) BV-2 cells were treated with PbO NPs at doses of 0.0, 2.5, 5.0 and 10.0 mg/L for 24 hours. iii) BV-2 cells were randomly divided into control group, PbO NPs group and triggering receptor expressed on myeloid cells 2 (TREM2) high expression + PbO NPs group. The cells in the control group received no treatment. The cells in PbO NPs group were exposed to 10.0 mg/L PbO NPs suspension for 24 hours. Cells in TREM2 high expression + PbO NPs group were transfected with Trem2 high expression plasmid, and then exposed to 10.0 mg/L PbO NPs suspension for 24 hours. iv) The mRNA expression of M1 markers [nitric oxide synthase (iNos), cyclooxygenase 2 (Cox2), chemokine receptor 7 (Ccr7)], M2 markers [arginin-1 (Arg-1), transforming growth factor-β (Tgf-β), chemokine receptor 2 (Ccr2)] and Trem2 of microglia was detected by real-time fluorescent quantitative polymerase chain reaction. The protein expression of iNOS, ARG-1 and TREM2 was detected by Western blotting. Results i) During the experiment, there was no significant difference in body weight of mice among these four groups (P>0.05). The relative expression of Cox2 and Ccr7 mRNA in the hippocampus of the mice increased in the low-dose group and the iNos, Cox2 and Ccr7 mRNA increased in the medium- and high-dose groups, compared with the control group (all P<0.05). The relative mRNA expression of Tgf-β in the hippocampus of the mice of low-dose group and Arg-1, Tgf-β and Ccr2 in the medium- and high-dose groups was decreased compared with the control group (all P<0.05). The mRNA relative expression of iNos, Cox2 and Ccr7 was increased (all P<0.05), while the mRNA relative expression of Arg-1, Tgf-β and Ccr2 was decreased (all P<0.05) in the hippocampus of the mice of high-dose group compared with the low-dose group. The relative expression of Trem2 mRNA and TREM2 protein in the hippocampus of mice of the medium- and high-dose groups was lower than those in the control group (all P<0.05). The relative expression of Trem2 mRNA and TREM2 protein in the hippocampus of mice of the high dose group was lower than those in the low- and the medium-dose groups (all P<0.05). With the increase of PbO NPs exposure dose, the relative expression of iNOS protein in hippocampus tissues of mice increased (P<0.01), and the relative expression of ARG-1 protein decreased (P<0.01). ii) With the increase of PbO NPs exposure dose, the relative expression of iNOS protein increased (P<0.01), and the relative expression of ARG-1 protein decreased (P<0.01) in BV-2 cells. The relative expression of iNOS protein in BV-2 cells of PbO NPs group and TREM2 high expression + PbO NPs group was increased (all P<0.05), and the relative expression of ARG-1 protein decreased (all P<0.05) compared with the control group. The relative expression of iNOS protein decreased (P<0.05), and the relative expression of ARG-1 protein increased (P<0.05) in BV-2 cells of TREM2 high expression + PbO NPs group compared with the PbO NPs group. Conclusion Exposure to PbO NPs could increase the M1 polarization and decrease the M2 polarization of microglia, with a dose-effect relationship. The M1 polarization of microglia decreased and M2 polarization increased after overexpression of Trem2 gene. The regulation of microglia polarization by TREM2 may be involved in the neurotoxic effects of PbO NPs.

RESUMO

Severe aplastic anemia (SAA) is a severe bone marrow failure syndrome caused by multiple causes, which is clinically manifested with severe anemia, infection and bleeding. The complex pathogenesis of SAA has not been fully understood. SAA is characterized with acute onset, severe disease condition and rapid progression. At present, with the in-depth study of SAA and the improvement of diagnosis and treatment, the therapeutic strategy for SAA has been evolved from classical immunosuppressive therapy based on antithymocyte globulin and cyclosporine to the application of thrombopoietin receptor agonist and combined treatment based on allogeneic hematopoietic stem cell transplantation, which may promote the reconstruction of hematopoietic function of SAA patients to varying degree and significantly improve survival and clinical prognosis, becoming the research hotspot of SAA treatment. In this article, new advances in the treatment of SAA at home and abroad were reviewed.

RESUMO

T cell immunoglobulin and immunoreceptor tyrosine-based inhibitory motif domain (TIGIT) is a new immune checkpoint protein. Studies have shown that TIGIT can cause dysfunction of immune cells, weaken the anti-tumor effect, thus leading to tumor immune tolerance and immune escape. Blocking TIGIT can reverse immune cell failure and exert anti-tumor effect, which is expected to become a new therapeutic target for multiple myeloma.

RESUMO

Objective To investigate the interfractional and intrafractional prostate motion during hypofractionated precise radiotherapy for prostate cancer. Methods From 2013 to 2016, twenty?eight patients who received 5 Gy radiotherapy in 9 fractions for prostate cancer were enrolled as subjects. Every patient had three gold fiducials implanted into the prostate by transrectal ultrasound guidance two weeks before computed tomography ( CT) simulation. All patients underwent CT scans in the supine position with full bladders and rectal balloons filled with 60 ml air. The Pinnacle planning system was used to design the treatment plans. Twenty?three patients were treated with a Synergy accelerator. Those patients underwent cone?beam CT ( CBCT) scans prior to treatment. The set?up error was recorded by bone alignment between CBCT images and planning CT images. The prostate displacement was then recorded by gold fiducial alignment. The interfraction prostate displacement was defined by the difference between the above two parameters. The other 5 patients were treated with a Novalis accelerator. Based on gold fiducial alignment, the real?time tracking of gold fiducials was carried out by the ExacTrac system to evaluate the intrafractional prostate displacement. Results A total of 207 measurements of interfractional prostate displacement were made in the 23 patients before treatment. The mean interfractional prostate displacements in the left?right (LR),superior?inferior (SI),and anterior?posterior (AP) directions were (0.05±0?10),(0.20±0?22),and (0.19±0?18) cm,respectively. The displacements larger than 0?3 cm in the above three directions were observed in 1,52,and 49 measurements, respectively, while the displacements larger than 0?5 cm in the three directions were observed in 1,29,and 16 measurements, respectively. A total of 225 measurements of gold fiducial displacement were made in the 5 patients during treatment. The mean intrafractional prostate displacements in LR,SI,and AP directions were (0.61±0?50),(0.68±0?69),and (0.70±0?67) mm, respectively. The displacements larger than 3 mm in the three directions were observed in 0, 1, and 1 measurement, respectively. Conclusions In hypofractionated precise radiotherapy for prostate cancer, the interfractional prostate displacement is significantly larger than the intrafractional prostate displacement. The interfractional prostate displacement must be corrected before radiotherapy. In order to avoid off?target irradiation due to postural changes in patients, the tracking of the intrafractional prostate displacement is still necessary although the displacement is relatively small. The rectal balloon can help immobilize the prostate.

RESUMO

Objective To analyze the influencing factors of cone-beam CT (CBCT) imagine registration in lung cancer. Methods From Mar. 2007 to Dec. 2007, 20 patients with lung cancer were treated with IGRT. The imagines of CBCT were collected from 6 to 19 fractions during the patients' radiotherapy. To compare the difference of set-up errors between the two groups according to the distance from the lesion in lung to the centrum. At the same time, CBCT imagines from the first, middle and the last fraction of these patients' radiotherapy were registrated in bone and grey methods by four doctors. The difference of set-up errors between different doctors and registrated methods were compared. Results The mean values of set-up errors were <2 mm in the two groups without significant difference (x:-1.31mm vs 0. 10 mm (t=0. 07,P=0.554);y:1.24 mm vs 1.37 mm (t=0. 05,P=0. 652);z: - 1.88mm vs -1.26mm (t= -0. 12,P=0.321)). The mean values of set-up errors were < 1.3 mm in four doctors and registrated methods without significant difference, for bone registration,x: -0. 05 mm, -0. 01 mm,0. 05 mm, -0.12 mm and -1.31 mm ( F=-0.01,P=0.887) ;y:0.56 mm,0.35 mm,0.51 mm and 0.43 mm (F= -0.01,P=0.880);z: -1.16 mm, -1.20 mm, -0.88 mm and -1.03 mm (F= -0.04,P=0. 555 ), for grey registration ,x: -0.32 mm, -0.341 mm, -0.395 mm and - 0.37 mm(F=-0.01, P=0.874);y:0.34 mm,0.54 mm, -0.04 mm and 0.27 mm (F= -0.03,P=0.622);x:-1.12 mm,- 1.15 mm, - 1.13 mm and - 1.04 mm (F=0. 00,P=0. 812). Conclusions With the same registrated box and imagine quality, the location of the lesions in lung, registred methods and different doctors are not the influencing factors for CBCT imagine registration.

RESUMO

Objective To evaluate the efficacy and toxicity of fractionated stereotactic body radiotherapy in treating patients with liver metastases. Methods From January 1997 to January 2007, 22 patients with liver metastases of total 33 lesions were treated with fractionated stereotactic body radiation therapy (SBRT). All patients were confirmed as unresectable and resistant to chemotherapy. The most common primaries were breast cancer in 12 patients and colorectal cancer in 5. Fifteen patients had metastases in the liver alone and 7 had concurrent metastases in other sites. Eighteen patients received one course of SBRT, and 4 patients got two courses. The median total dose was 40 (range, 30 - 60) Gy in 3 (range, 2 -6) fractions. Results All patients were followed up. The median follow up was 13.9 months (range, 5.0 -36. 4 months). The median size of the irradiated lesions was 2. 0 cm (range 1.0 -4. 3 cm) and the target volume was 3. 68 cm~3 (range 0. 45 - 77. 29 cm~3). The 1- and 2-year local control rate, overall survival rate and progression-free survival rate were 100% and 90%, 73% and 49%, 94% and 28%, respectively. No grade 3/4 SBRT related toxicities occurred. Conclusions SBRT, with high local control and tolerable complications, is an effective and safe treatment for liver metastases.

RESUMO

Objective: To study the effect of wortmannin (WM), a PI3K/Akt inhibitor, on the proliferation and apoptosis of leukemia cells and the possible mechanism. Methods: Human leukemia cell line K562 was treated with different concentrations of WM. The proliferation of K562 cells was examined by MTT assay. DNA damage in K562 cells was examined by single cell gel electrophoresis assay, and apoptosis of K562 cells was detected by Annexin V-FITC/PI double-staining. The expressions of total Akt, phosphorate-Akt (p-Akt), and NF-κB p65 mRNA and protein were detected by RT-PCR and Western blotting, respectively. Results: WM inhibited the proliferation of K562 cells in a dose- and time-dependent manner, with the IC((50) value of 24 h being 25 nmol/L. WM also induced apoptosis of K562 cells in a dose-dependent manner. DNA damage in K562 cells was demonstrated by appearance of comet tail after treatment with WM, with the rate of DNA tail and the tail length being significantly higher than those in the control group (P<0.01). WM dose-dependently inhibited P-Akt and NF-κB p65, but not the total Akt, mRNA and protein expressions. Conclusion: WM can inhibit proliferation and induce apoptosis of K562 cells in a dose- and time-dependent manner, probably through down-regulation of phosphorate PI3K/Akt signal pathway and NF-κB expression.

RESUMO

Objective To study the value of image-guided radiotherapy (IGRT) in lung cancer. Methods From Mar. 2007 to Dec. 2007,58 patients with lung cancer were treated with IGRT. Set-up er-rors in each axial direction was calculated based on IGRT images of each patient. The change of GTV was e-valuated on both cone-beam CT and CT simulator images. Results Twenty-two patients with left lung cane-er,30 with right lung cancer,5 with mediastinal lymphanode metastasis and one with vertebra metastasis were included. The set-up error in x,y and z axes was (0.02±0.26) cm, (0.14±0.49) cm and ( -0. 13± 0.27) cm, respectively, while the rotary set-up error in each axis was -0.15°± 1.59°, -0.01°± 1.50° and 0.12°±1.08°, respectively. The set-up errors were siguifieantly decreased by using of IGRT. GTV movement was observed in 15 patients (25.9%) ,including 5 with left upper lung cancer. GTV moving to the anterior direction was observed in 9 patients, including 4 with]eft upper lung cancer. GTV reduced in 23 (44.2%) patients during treatment. Asymmetric GTV reduction of 22 lesions was observed, with a mean re-ductive volume of 4.9 cm3. When GTV began to shrink, the irradiation dose was 4 -46 Gy,with 20 -30 Gy in 9 patients. Conclusions The use of IGRT can significantly reduce set-up errors. GTV movement and reduction are observed in some cases. The time to modify the target volume needs to be further studied.

RESUMO

Objective To evaluate the systematic and random set-up errors in patients immobilized with thermoplastic device during radiotherapy, and to determine the proper margins extended from clinical target volume ( CTV ) or internal target volume ( ITV ) to planning target volume ( PTV ). Methods From March 2007 to September 2007,120 patients were included in this study, including 13 receiving head and neck irradiation, 67 thoracic irradiation and 40 abdominal irradiation. All patients were immobilized with thermoplastic device and received CT simulation and intensity modulated radiation therapy(IMRT). X-ray cone beam CT was regularly performed before treatment and the images were compared with the simulation CT images. The shift and rotation in right-left( R-L), superior-inferior(S-I) and anterior-posterior(A-P) directions were recorded and analyzed. The shift margin from CTV or ITV to PTV was calculated with the equation, margin = 2'mean + 0.7'standard deviation. Results In head and neck region, the shift errors in R-L,S-I and A-P directions were(0.13 ±0.15) cm, (0.13 ±0.17) cm and(0.11 ±0.14) cm,and the corresponding rotation errors were 1.05°± 0.77°,0.87°± 1.13° and 0.68°±0.89°. The margins from CTV to PTV were 0.37 cm,0.38 cm and 0.31 cm,respectively. In thoraci region, the shift errors in R-L,S-I and AP directions were(0.20 ±0.27) cm, (0.34 ±0.44) cm and(0.25±0.31 ) cm,and the corresponding rotation errors were 1.06°±1.45° ,0.85°±1.23° and 0.78°±1.08°. The shift margins from ITV to PTV were 0.59 cm, 1.00 cm and 0.72 cm. In abdominal region, the shift errors in R-L, S-I and A-P directions were (0.23 ± 0.30) cm, (0.37 ±0.45 ) cm and ( 0.27 ±0.34 ) cm, and the corresponding rotation errors were 1.22°±1.56°, 1.05°± 1.44°and 0.98°± 1.24°. The shift margins from CTV or ITV to PTV were 0.66 cm, 1.05 cm and 0.78 cm. Conclusions Cone beam CT can be used in the precise measure of set-up errors, which can provide institution-specific margins for PTV designing in patients immobilized with thermoplastic device.

Objective To investigate the effect of dendritic cells(DC)pulsed with tumor lysate antigens of K562/A02 co-cultured with cytokine induced killer(CIK)cells on cytotoxicity against multidrug resistant(MDR)leukemia K562/A02 cells with high expression of p-glycoprotein(P-gp).Methods Bone marrow mononuclear cells isolated from healthy adult donors were induced to obtain CIK cells and DC respectively and then CIK were co-cultured with DCs which were pulsed with tumor lysate. The morphologic features were observed with optical microscopy and expressions of markers typical were measured using flow cytometry. The cytotoxicity of three effect cells was measured with MTT assay respectively. Results After pulsing DCs with tumor lysate antigens co-cultured CIK or DC-CIK led to a significant increase of lymphocyte proliferation of both populations. The cytotoxicity of CIK co-culturing with DCs pulsed with tumor lysate antigens to K562/A02 and K562 cells was(42.90?0.67)%、(49.85?0.28)%(63.36?0.46)% and(23.56 ?0.43)%、(26.11?0.34)%、(34.46?0.35)% respectively at an effector-to-target ratio of(5∶1、 10∶1 、20∶1).Conclusion This study indicates that DC pulsed with tumor lysate antigens co-cultured with CIK can improve lymphocyte proliferation and enhance the cytotoxic activity.Especially,it can induce a P-gp specific anti-leukemic immune.

RESUMO

Objective To investigate the effect of treatment for la rg e-sized auricular nevus through split-thickness grafting over perichondrium. Methods In the six cases with large-sized and giant cong enital melanocytic nevi of the auricle, the auricular lesions were radically exc ised in stages, and each time followed by split-thickness skin grafting. The ou tcome was evaluated. Results The grafts completely surviv ed. During the time of follow-up ranging from one to two years, the grafts expe rienced no contraction, and auricular deformity did not occur. Conclu sion Treatment of large-sized auricular nevus through staged split -thickness skin grafting over perichondrium is simple and reliable.
